EmpowerRF & the O-Shot:
The Treatments We Need
to Talk About More
You've been quietly dealing with it for years. The leak when you laugh too hard. The dryness that makes intimacy something you dread instead of enjoy. The way you plan your day around bathroom access. The slow disconnection from your own body that you've convinced yourself is just part of getting older. It isn't. And there's a reason no one told you that.
We don't talk about these things. Not to our providers. Not to our friends. Sometimes not even to ourselves. There's a particular cruelty in conditions that are both common and invisible — conditions that affect millions of women but are whispered about, brushed off, or normalized into silence. Urinary incontinence. Vaginal dryness and atrophy. Painful sex. Diminished sensation. Reduced libido. Pelvic floor weakness. These are not character flaws. They are not the price of womanhood. They are treatable physiological conditions — and the treatments available right now, in-office, without surgery or downtime, are extraordinary.
I'm talking about EmpowerRF by InMode and the O-Shot. And if you haven't heard of them, that tells you everything about how we fail women in this conversation.
Why We Don't Talk About This
(And Why That Has to Change)
The statistics alone should end the silence. Studies show that urinary incontinence affects up to 44.6% of women1 — nearly half. A different population study found that 95.5% of women with incontinence reported a negative impact on their quality of life1, yet 64.7% never received any medical help for it. Read that again: almost two-thirds of women suffering from a condition that's disrupting their daily life haven't told a single healthcare provider. Because they're embarrassed. Because they think it's normal. Because they've been told, implicitly or explicitly, that leaking is just what happens after you have babies or after a certain age.
And it's not just incontinence. Genitourinary Syndrome of Menopause (GSM) — the umbrella term for the cascade of tissue changes that happen when estrogen declines — affects somewhere between 40 and 70% of postmenopausal women.2,3 GSM means vaginal dryness. It means pain with sex. It means urinary urgency, recurrent infections, changes in the tissue you live in every day. And it's progressive — it gets worse over time unless it's treated.
The cruel irony is that GSM gets harder to talk about the more symptomatic you become. A 2016 review in the American Journal of Obstetrics & Gynecology found that only 25% of women with GSM ever consult a healthcare provider about it.4 Only 4% of women were even able to attribute their symptoms to GSM. The language doesn't exist in most clinical settings to have this conversation clearly — and so it doesn't happen at all.
"These symptoms are not inevitable. They are not the price of aging. They are treatable physiological conditions — and the gap between how many women suffer and how many women get help is one of the most fixable problems in women's healthcare."
— Heather Mateja, NP-C · Atlantic Aesthetics MaineThis is where EmpowerRF and the O-Shot come in. Not as luxury treatments. Not as vanity procedures. As clinical tools that address real physiological problems — tools backed by FDA clearance, mechanism-of-action research, and the lived experience of women who describe them as life-changing.
What Is EmpowerRF?
A Platform, Not a Single Treatment
EmpowerRF by InMode is not a single device. It's a multi-technology platform housing several FDA-cleared modalities — each targeting a different aspect of feminine wellness — that can be used individually or in combination depending on your specific symptom picture. This is the key distinction from older approaches to vaginal rejuvenation: EmpowerRF treats the whole woman's wellness picture, not just one complaint in isolation.
At Atlantic Aesthetics, we use three EmpowerRF modalities: VTone, FormaV, and Morpheus8V. Here's what each one does and why it matters.
VTone
Electrical Muscle Stimulation · Pelvic FloorDelivers intravaginal EMS to rehabilitate and strengthen pelvic floor musculature. FDA-cleared for stress, urge, and mixed urinary incontinence. Think of it as medically supervised Kegel training — except far more effective than what you'd do on your own, because the device directly stimulates the muscle to contract with precision and consistency. Six weekly sessions are standard protocol.
FormaV
Bipolar RF · Tissue RemodelingDelivers controlled bipolar radiofrequency energy deep into vaginal and labial tissue, creating uniform volumetric heat that stimulates collagen production and tissue contraction. Addresses dryness, laxity, atrophy, and tissue quality — both internally and externally. Comfortable enough that most patients describe it as a warm internal massage. Three sessions, four weeks apart.
Morpheus8V
RF Microneedling · Sub-Epithelial RemodelingThe most advanced of the three: fractional microneedling combined with RF energy, penetrating up to 3mm below the vaginal epithelium to trigger deep structural collagen remodeling. Addresses moderate-to-advanced vaginal laxity, significant tissue atrophy, and dryness that hasn't fully responded to RF alone. Topical numbing applied beforehand. Three sessions, four to six weeks apart.
Combination Protocols
Personalized Treatment PlansBecause pelvic floor muscle strength and vaginal tissue quality are related but separate concerns, most women benefit from combining modalities. VTone and FormaV are often done in the same visit. The packages we've developed at Atlantic Aesthetics are designed around the most clinically meaningful combinations — starting at the level that matches your symptoms, not a one-size-fits-all script.
The Science Behind the Technology
These aren't unproven devices. The mechanisms are well-understood, and the clinical application of RF energy and EMS for pelvic and vaginal health has been studied for over a decade. Radiofrequency energy works by delivering controlled thermal injury to tissue — not damage in the harmful sense, but the controlled cellular stress that triggers the body's natural repair cascade: collagen synthesis, neovascularization, tissue contraction, and regeneration of the vaginal epithelium. This is the same mechanism that makes RF so effective on the face for skin tightening, except applied intravaginally where the same collagen and elastin depletion is occurring as a result of estrogen decline.
VTone's EMS mechanism works similarly to the electrical stimulation used in physical therapy for muscle rehabilitation — except the intravaginal approach allows the device to target the levator ani and pelvic floor muscle group directly, with far greater precision than surface-level stimulation or voluntary exercise alone. The result is functional muscle rehabilitation that translates to meaningful reduction in incontinence episodes.
The O-Shot:
PRP for Intimate Tissue Regeneration
The O-Shot (Orgasm Shot) is a PRP-based treatment — platelet-rich plasma derived from your own blood — injected into the vaginal wall and clitoral tissue to stimulate cellular regeneration from within. It addresses the biological side of intimate wellness: tissue sensitivity, arousal, natural lubrication, and mild urinary incontinence, by rebuilding the tissue at the cellular level rather than just remodeling its structural architecture.
PRP contains concentrated growth factors — PDGF, VEGF, TGF-β, and others — that signal the body to initiate new collagen formation, improve blood flow, and regenerate cellular activity in tissue that has become atrophic or hypovascular. In the context of vaginal and clitoral tissue, this means improved nerve sensitivity, better natural lubrication response, enhanced arousal, and in many cases meaningful reduction in mild stress urinary incontinence.
What the Research Shows
A prospective study published in the Journal of the American Medical Association and indexed in PubMed examined PRP administration to the anterior vaginal wall in women with sexual dysfunction and orgasmic disorder. Results showed that orgasm subdomain scores increased from 2.11 to 4.48 following PRP treatment (p<0.001), with the total Female Sexual Function Index score improving significantly in 50% of patients.5 The authors concluded that PRP has regenerative potential for improving female sexual function through collagen formation and neovascularization in vaginal tissue.
A 2023 systematic review published in PMC examining PRP for female sexual dysfunction and stress urinary incontinence found that PRP improved symptoms across multiple validated measures including the Female Sexual Function Index (FSFI), with generally favorable safety profiles and minimal adverse effects.6
A 2025 scoping review in the Journal of Clinical Medicine examining PRP across gynecological disorders found that PRP therapy showed improvements in sexual interest, activity, and pleasure in women with sexual dysfunction, and was generally well-tolerated with only minor adverse effects such as transient injection-site discomfort.7
Why These Treatments Work
Better Together
EmpowerRF and the O-Shot address the same problem from different angles — and that's precisely why the combination is often more powerful than either alone.
RF-based treatments (FormaV and Morpheus8V) work on the structural architecture of vaginal tissue: stimulating collagen, tightening laxity, improving the physical condition of the tissue. VTone works on the functional neuromuscular side: strengthening the muscles that support the pelvic floor and bladder. The O-Shot works at the cellular and biological level: flooding tissue with growth factors that rebuild vascularity, sensitivity, and cellular vitality from within.
Think of it this way:
RF remodels the architecture. EMS rebuilds the muscle function. PRP regenerates the biology. Each works through a distinct mechanism. Used together, they address what aging, childbirth, and hormonal change do to a woman's body at every level — structural, functional, and cellular.
This is why our most comprehensive package, the Full Wellness Program, exists. It's not upselling. It's clinical logic: each modality does something the others don't, and most women dealing with the full picture of GSM, incontinence, and sexual wellness changes benefit from addressing all three dimensions.
Who Is This For?
More Women Than You Think
The honest answer is: far more women than are currently seeking treatment. EmpowerRF and the O-Shot are not treatments reserved for a specific life stage or severity level. They're appropriate for:
- Postpartum women with pelvic floor changes
- Perimenopausal women experiencing early tissue changes
- Postmenopausal women with GSM symptoms
- Women with stress, urge, or mixed urinary incontinence
- Women experiencing vaginal dryness or atrophy
- Women with pain during intercourse (dyspareunia)
- Women with reduced sensation or arousal
- Women with decreased libido
- Women who want to proactively maintain tissue health
- Women who are not candidates for or prefer to avoid hormonal therapy
That last point matters. EmpowerRF and the O-Shot are entirely non-hormonal. For women who can't use estrogen — breast cancer survivors, women with hormone-sensitive conditions, women who simply prefer to avoid it — these treatments offer a meaningful path to relief that doesn't require hormonal intervention. They can also work beautifully alongside HRT for women who are already on it, addressing the structural and functional changes that hormones alone don't fully reverse.
What to Actually Expect
No Surgery. No Downtime. Real Results.
Everything happens in-office. There's no anesthesia, no incisions, no recovery period that takes you off the court for days or weeks. VTone and FormaV are comfortable enough that patients bring a book or scroll their phones during treatment. Morpheus8V — the deepest of the three — requires topical numbing applied beforehand, and most patients describe it as manageable pressure rather than pain. The O-Shot involves a topical numbing cream prior to injection and is very well-tolerated.
Downtime is minimal across the board. We ask that you avoid sexual intercourse and strenuous exercise for 48 to 72 hours after RF treatments, and a full week following Morpheus8V. Beyond that, you return to your day immediately. Most of our patients come in on their lunch break.
Results develop gradually — and that's actually a good sign. The collagen remodeling and cellular regeneration triggered by these treatments build over weeks, with the most significant improvements typically visible four to eight weeks after completing the series. Many women, especially with VTone for incontinence, notice meaningful changes after the very first session. Full results from the complete series continue to improve for several months after treatment ends, and typically last 12 to 18 months before a maintenance session is recommended.
The Conversation We Should Be Having
Here's what I want every woman reading this to take away: the silence around these symptoms is a choice we're all making collectively, and it has real consequences. Women are wearing pads unnecessarily. Women are avoiding intimacy with their partners. Women are declining physical activities they love because of leaking. Women are enduring painful sex in silence because they've been told this is just how it is now.
It's not how it has to be.
I built our Women's Intimate Wellness program at Atlantic Aesthetics because I kept seeing patients — in my medical background and in aesthetics — who had been quietly suffering with these symptoms and had never once been offered a real option. Not because the options didn't exist. Because no one asked, no one offered, and the cultural discomfort around "vaginal" anything kept these conversations from happening in clinical spaces where they absolutely should.
Your consultation at Atlantic Aesthetics is private, pressure-free, and without obligation. We look at your symptoms, your goals, your timeline, and your health history — and we build a plan that makes clinical sense for your specific picture. You don't have to come in having already decided what you want. You just have to come in.
